State of Ohio employees and/or their spouses who are enrolled in Take Charge! Live Well! and who had a wellness activity process in the month of October should see the incentive on the Friday, December 6 paycheck. If a member is no longer employed by the State at the time the reward was generated, a separate check will be issued to the agency for distribution.
State employees and their spouses are eligible to receive Take Charge! Live Well! rewards if enrolled in the State's medical plan. Take Charge! Live Well! is a joint labor and management health care initiative designed to let employees and families take their health and wellness into their own hands. Enrollees can each earn up to $350 by completing activities to improve your health and wellness.
