Verify your State Health Plan dependents by April 5

The State of Ohio employee dependent verification audit is underway. This includes dependent verification for medical, dental, vision and life insurance. Employees, including all OCSEA state bargaining unit members, have until April 5, 2019 at 10 p.m. EST to submit required dependent documentation. Dependents who are not verified by the deadline will be dropped from the State of Ohio Health Plan effective May 1, 2019 and will not be eligible for COBRA.

The vendor selected to perform the verification audit is Alight Solutions. Recent correspondence from OhioDAS includes your unique Dependent Verification ID. Visit this LINK to complete your verification.  

To ensure the privacy of employees, all private, financial information should be redacted on documentation showing proof of joint ownership for spouses and domestic partners. For those without proof of joint ownership (i.e., house is only in one person's name or there are separate bank accounts, etc.), Alight will accept a copy of each individual's bank statement. The following proof must be submitted:

  • Date (within 6 months)
  • Logo of the company (bank, utility, mortgage company, etc.)
  • Names of both member and spouse
  • Address on file

Please send copies only, NOT originals. Documents can be scanned or photographed on your mobile phone for easier submission.
